   DOCUMENTS REQUIRED FOR FILING INSURANCE CLAIM












   Medical Expense

  • Original claim form duly signed and filled in.
  • Air ticket jacket / boarding pass / Passport copy with entry & exit stamp.
  • Doctors statement mentioning the ailment and treatment & medication prescribed for the same.
  • Original bills received from the hospital for services rendered by them.
  • Original payment receipts indicating that the amounts due to the hospitals have been fully settled.

   Delay Of Baggage

  • Original claim form duly signed and filled in.
  • Air ticket jacket / boarding pass / Passport copy with entry & exit stamp.
  • Property Irregularity Report issued by the airlines.
  • Original Receipts for emergency purchases made during the period when baggage was delayed.
  • Letter from airline if any compensation is paid to the insured towards Settlement of claim for “Delay Of Baggage”.

   Dental Treatment

  • Original claim form duly signed and filled in.
  • Air ticket jacket / boarding pass / Passport copy with entry & exit stamp.
  • Doctors statement mentioning the ailment and treatment & medication prescribed for the same.
  • Original bills received from the hospital for services rendered by them.
  • Original payment receipts indicating that the amounts due to the hospitals have been fully settled.

   Loss Of Baggage

  • Original claim form duly signed and filled in.
  • Air ticket jacket / boarding pass / Passport copy with entry & exit stamp.
  • Property Irregularity Report issued by the airlines.
  • Letter from airline confirming that the baggage has been permanently lost And if any compensation is paid towards settlement of claim.
  • Description of items lost along with relevant bills (bills are must for items in excess of $ 100)

   Loss Of Passport

  • Original claim form duly signed and filled in.
  • Air ticket jacket / boarding pass / Passport copy with entry & exit stamp.
  • Police report.
  • Receipts for obtaining duplicate passport / travel documents (Eg Payment Made to embassy of India for making duplicate passport, Emergency Certificate issued by embassy of India., stamps, cost of a professional account or solicitor, Excluding any transportation involved in getting duplicate passport.

   Trip Curtailment/Cancellation

  • Original claim form duly signed and filled in.
  • Air ticket jacket / boarding pass / Passport copy with entry & exit stamp.
  • Original bills of expenses incurred due to cancellation/Curtailment.
  • Copies of cancellation correspondence with airline authorities, hotel, car rental and tour operator certifying the cancellation, along with details of compensation received from airlines / other authorities (if any)
  • Proof of the reason for cancellation/Curtailment.
  • Details of refund received from the airline, tour operator & hotel booking.

   Repatriation of Mortal Remains

  • Original claim form duly signed and filled in.
  • Policy copy.
  • Air ticket jacket / boarding pass / Passport copy with entry & exit stamp.
  • Doctors statement mentioning the illness, its duration, investigation and treatment prescribed for the same.
  • Original bills received from the undertaker/ funeral for services rendered by them.
  • Original Death certificate/ Coroner’s report received from the hospital.
  • Certificate of clearance from the Indian Embassy.

   Personal Accident International

  • Original claim form duly signed and filled in.
  • Policy copy.
  • Air ticket jacket / boarding pass / Passport copy with entry & exit stamp.
  • Police Report.
  • Doctor’s Statement or the medical report mentioning the cause of he death or disability of the insured.

   Financial Emergency

  • Original claim form duly signed and filled in.
  • Policy copy.
  • Air ticket jacket / boarding pass / Passport copy / E-Ticket jacket.
  • Detailed police report (In English).
  • Detailed FIR report (In English).
  • Brief description of incident & details of items lost.

   Trip Delay

  • Original claim form duly signed and filled in.
  • Policy copy.
  • Air ticket jacket / boarding pass.
  • Original bills/invoices of expenses incurred during delay period.
  • Copies of correspondence with the provider (Airline, hotel and tour operator).
  • Details of compensation received if any.
  • Proof (Reason) for trip delay.
